CCI Health Services is looking for a Psychiatric Nurse Practitioner to support behavioral health care by providing medication management, assessments, and treatment primarily to children, adolescents, and young adults with consultations to CCI Medical Providers at all locations. The Psychiatric Nurse Practitioner (PNP) provides direct patient care commensurate pertaining to his/her training and experience, and in collaboration with CCI physicians. The care provided is appropriate for an out-patient, primary care community health center with a multicultural, multilingual patient population.
As a member of the health center’s integrated care team the Psychiatric Nurse Practitioner will fulfill the following KEY FUNCTIONS AND RESPONSIBILITIES towards the provision of patient-centered, coordinated patient care:
- Examine diagnose, and treat patients within the parameters of his/her licensure, training, and experience.
- Provide direct consultation for complex patients, making recommendations to providers.
- Provide time-limited Stabilizing care for complex or high-risk clients.
- Consults on behavioral health systems and processes.
- Teaches CCI providers and staff about behavioral health topics such as psychopharmacology, diagnosis, treatment, management of challenging behaviors, etc.
- Promotes an atmosphere of teamwork and excellence among staff. Meets with program staff on a regular basis.
- Works collaboratively with all providers to ensure coordinated patient-centered activities.
- Provides telephone, electronic, and/or in-person consultation with providers about patient issues. These issues can include diagnostic clarification, treatment recommendations, or other issues.
- Primary focus is for children and adolescents - ages 6 to 21 with overlap with Adult care if needed.
- Adhere to evidence-based medical guidelines and standards approved by CCI’s Board and Performance Improvement Committee.
- Complete electronic medical record documentation in expected time frames.
- Participate in peer review and performance improvement activities and audits.
- Attend and participate in meetings and/or committees as necessary and/or required by CCI.
- May provide consultation to other providers or employees at CCI.
- May instruct nursing students in clinical practice and procedures.
- Participates in CCI programs and partnerships as appropriate.
- Participates in additional medical projects as assigned.
- Other duties or responsibilities as assigned by the Associate-CMO or designee.
KNOWLEDGE, SKILLS, AND ABILITIES:
- Knowledge of state regulations governing advanced practice nursing.
- Skill in communicating effectively with patients and their families.
- Skill in establishing and maintaining effective working relationships with other employees, patients, and the general public.
- Knowledge/experience with Electronic Medical Records (EMR) is a plus.
MINIMUM QUALIFICATIONS:
- Nurse Practitioner certification from an accredited program
- Active license issued by the State of Maryland Board of Nursing
- Active Maryland CDS license. DEA licensure.
- Bi-lingual in English, Spanish, or another second language preferred.
- Experience working with Interpreters through Language line services
- Ability to relate to culturally diverse patients and community.
- Flexibility to work at different CCI locations when needed.
