Join Choptank Community Health’s growing integrated behavioral health program and work as part of a multidisciplinary team, of therapists, medical and dental providers, focused on whole patient wellness.
Full time, part time, hybrid, and flexible schedules available. Caseload requirements for our providers are 9 patients per day Monday-Friday, and with no on call requirements.
Choptank Health is a Federally qualified Health Care facility and participates with the National Health Service Corp loan repayment program. Qualified clinicians can be awarded up to 100,000 dollars.
SUMMARY
Under the direction of the Director of Behavioral Health, and as a part of a multidisciplinary team the Behavioral Health Clinician will provide direct services including individual (children and adults), couple, family, and group therapeutic services to patients.
ESSENTIAL DUTIES
Addresses referrals from Choptank Health Medical and Dental providers, including referrals via warm hand-offs
Participates in multidisciplinary meetings to assess needs and coordinate services
Maintains weekly scheduled appointments in our electronic records system
Provide therapeutic services to patients and their families in collaboration with multidisciplinary team members including external partners
Completes required documentation and implementation in a timely manner. Documentation includes intake paperwork, biopsychosocial assessment, diagnostic screens and assessments as necessary, treatment planning, discharge planning, satisfaction survey, and community referrals when needed.

Maintain an active client caseload, based on needs of program and in accordance with COMAR regulations.

NECESSARY SPECIAL REQUIREMENTS
Working knowledge of MD Mental Health policies and procedures.
Knowledge and experience with evidence-based practices, including trauma informed care framework.
Strong clerical/administrative skills including personal computer skills (Microsoft Office software).
