POSITION SUMMARY:
Provide effective culturally competent behavioral health assessments and therapy to individuals, families and groups in a community-based and in-person primary care setting. Works flexibly as part of an integrated primary care team and promotes integrated behavioral health.
As a member of the health center’s integrated care team the Behavioral Health Consultant will fulfill the following KEY FUNCTIONS AND RESPONSIBILITIES towards the provision of patient-centered, coordinated patient care:
Therapist:
Maintain confidentiality of records relating to patients'
Provide mental health assessment.
Provide individual, couples and family with mental health treatment for adults, children, and teens using evidence-based interventions such as CBT, MI, trauma informed care and/or solution-focused brief therapy, and harm reduction and recovery.
Develop and implement treatment plans based on patients’ expressed goals.
Complete and maintain client- related records in the EMR (ECW), daily.
Follow regulations for documentation and
Collaborate and/or consult with other team members to perform clinical assessments or develop treatment
Coordinate clinical approaches with the medical
Modify treatment activities and documentation to comply with changes in patient’s status for accreditation or regulatory requirements.
Counsel family members to assist them in understanding, or supporting patients, as
Monitor patients’ use of medications and encourage their collaboration with
Provides office-based in-person and virtual appointments.
Additional Responsibilities:
Work alongside primary care providers to promote whole person care.
Be available for Warm Hand-offs
Provide mental health and substance abuse assessments to assess readiness and treatment length (short term vs Long-term)
Provide short-term mental health treatment for adults, children and teens using Brief and Evidence-based interventions.
Conduct substance abuse screenings and brief counseling or referral for addictions
Collaborate and/or consult with other Providers to develop unified treatment
Coordinate clinical approaches with the medical
Support family members to assist them in understanding, or supporting patients, as appropriate and refer to community resources or to specialists that are beyond CCI Scope of Practice.
Ability to provide crisis intervention services to patients, an ability to be flexible, use new ideas, and operate in a fast-paced health

KNOWLEDGE, SKILLS, AND ABILITIES:
Must be a Licensed Clinical Social Worker (LCSW or LCSW-C), Licensed Master Social Worker (LMSW), Licensed Clinical Professional Counselor (LCPC), Licensed Graduate Professional Counselor (LGPC), or Licensed Psychologist
Minimum 2 years providing therapy to children, teens, and adults with mental health needs.
Excellent team-based communication skills with the primary care team, supervisors, and other CCI staff.
Ability to define problems, collect and interpret data, establish facts, draw valid conclusions, and process work to completion.
Bi-lingual (Spanish, Amharic, Pashto) in reading, writing, and speaking, preferred
Experience in the use of interpreter services.
Ability to develop therapeutic rapport and engage patients in treatment using clinical skills including motivational interviewing in a culturally-based manner.
Ability to provide crisis intervention services to patients, and ability to be flexible, use new ideas and operate in a fast-paced health environment
Prior experience collaborating with physicians and other health care providers, social service agencies, and/or education systems desired
Enthusiasm for innovation, data-driven decisions, and clinical and financial outcomes.
