Position Summary:
Reporting to the CMO, the Medical Director of Population Health, Clinical Quality and Access will oversee clinical quality initiatives at CCI, ensuring the highest standards of care while optimizing patient access. This role will focus on improving health outcomes, reporting requirements of clinical metrics, and optimizing the EMR to improve efficiencies across the organization. The Medical Director will also oversee all nursing staff, including nurse managers and lead nurses, fostering a collaborative environment that promotes the highest standards of patient care.
Develop and then oversee the successful implementation of strategies to address population health. This may include identifying opportunities for improvement in chronic disease management, preventative care and health screening, especially in high-risk populations.
Analyze data, with the support of the Health Information Technology team, to identify and address care gaps using all available clinical resources.
Coordinate efforts to optimize community health strategies and value-based care with external stakeholders.
Clinical Quality Leadership
Oversee the development, implementation, and evaluation of our annual clinical quality plan and other initiatives across CCI, ensuring alignment with organizational goals and regulatory requirements.
Monitor and foster the achievement of clinical quality measures and reporting structures among clinicians.
Drive population health management strategies and ensure standardization and compliance with quality measures and established workflows.
Quality Improvement Initiatives
Monitor, analyze and report to relevant stakeholders progress on clinical quality metrics established for but not limited to UDS, HEDIS, and Value-Based care programs and chart agreed upon strategies as necessary.
Ensure compliance and accurate documentation for quality requirements set by funders, state and national accreditation bodies.
Plan for, oversee and respond to any annual External Quality Review Organization reviews, JACHO, DHHS, PCMH, HRSA, SAMHSA as assigned.
Facilitate quality improvement projects using industry established methodologies to enhance clinical workflows and patient outcomes.
Track, present results and drive needed to change for successful achievement of key performance indicators (KPIs) related to clinical quality and access.
Present updates on clinical quality initiatives and progress to staff, leadership, and external partners.
Perform probationary, and other peer reviews, including chart reviews for department reports as needed.
Data Analysis and EMR Optimization
Work closely with HIT, the Operations department and EMR specialists to develop efficient workflows, standardize documentation, and leverage AI to improve clinical processes.
Implement and foster the successful use of emerging, evidenced based and/or problem-solving strategies to reach data targets.
Patient Access and Virtual Care
Oversee after-hours and open-access scheduling, coordinating scheduling for telehealth services to ensure that virtual care services are effectively integrated into the patient care model.
Partner with clinical operations to optimize patient access and utilization goals.
Work with the HIT team to enhance patient portal access, improving communication between patients and their care teams to facilitate timely and effective care.
Nursing Leadership
Supervise nursing manager and oversee the nursing department, fostering a culture of teamwork and excellence in clinical practice.
Patient Care
Performs direct patient care and executes treatment plans according to evidence-based treatment guidelines.
Conducts comprehensive medical histories, completes screenings, patient education and executes CCI standing orders.
Implements care coordination and management for individuals living with chronic conditions such as diabetes, hypertension, hyperlipidemia and HIV.
Develops care plans, addresses barriers to care, coordinates transitions of care, ensures access to medication, and supports the referral process.
Maintains a panel of patients receiving care– coordinating follow-up, labs, prescriptions and or administration of injectable medications per CCI standing orders.
Adheres to evidence-based care practice guidelines and CCI standards of professionalism.
Participates in quality assurance, quality improvement, data reporting and workflow analysis projects as assigned.
Maintains compliance with state and federal license and educational mandates and seeks additional educational opportunities to enhance professional competence.
Maintains the patient’s electronic medical record within expected timeframes, following CCI standards for charting.
Provides quality customer service and communicates in a HIPAA compliant, patient-centered, and culturally sensitive manner.
Attends and participates in meetings, committees, health fairs, events and training as assigned during standard work hours.
Performs other responsibilities as assigned by CCI leadership.
Works within the policies and procedures of CCI Health Services, Inc.
MINIMUM QUALIFICATIONS:
Active RN or LPN (RN preferred) license issued by the State of Maryland Board of Nursing or any state under the Nurse License Compact.
Current BLS/CPR certification required
Minimum of 3 years of leadership experience (5 preferred) with a focus on population health, quality improvement and patient access.
Minimum of 3 years of clinical experience and desire to continue clinical practice.
Strong understanding of quality improvement methodologies and data analysis.
Excellent verbal and written communication skills. Additional fluency in Spanish is preferred.
Understanding of Word, Excel and Teams.
Strong diagnostic, analytical, problem-solving, and critical thinking skills.
Ability to collaborate effectively with diverse, multidisciplinary teams.
High level of customer service focus.
Knowledge of current technological trends in healthcare, particularly in EMR optimization and virtual care.
